American Red Cross Hometown Heroes
2013 NOMINATION FORM
Submission Deadline: February 25, 2013
Do you know a Hometown Hero? Heroes can be hard to classify, but
each displays — courage, dedication, concern, or a conviction — something that
moves and inspires us to do more and to be more.
On Thursday, March 14, 2013,
the
American Red Cross will recognize individuals exemplifying courage, kindness
and unselfish character at our annual luncheon. Five local heroes will be
selected from nominations submitted by the community. These outstanding individuals should
fall into one of the following categories: 1) Good Samaritan – Youth (18 and younger); 2)
Good Samaritan – Adult; 3) Red Cross Volunteer-Hospital; 4) Red Cross
Volunteer-Disaster; 5) First Responder.
Please complete and return the form, along with a one page, double-spaced
typewritten statement about your nominee.
Use the essay to tell a compelling story about why your nominee should
be selected.
Nomination forms must be received by 4:45 PM, February 25, 2013. Nomination forms become the property of the
Red Cross and will not be returned.
The Hometown Hero judging committee will be comprised of civic and
community leaders. All nominations will
be verified by a representative of the Red Cross. Decisions are final.
Nominees must be from
Hamilton, Bledsoe, Grundy, Sequatchie, Rhea, Marion, Bradley, Meigs, McMinn,
Polk or Monroe counties. If selected, nominees
must be available for promotions events and able to attend the Heroes luncheon,
Thursday, March14, 2013, 11:30-1:00, at The Chattanoogan. The Red Cross will
host each finalist and one guest (two guests for Youth finalists) at the
luncheon.
All finalists must
sign information release forms authorizing the Red Cross unlimited permission
to use, publish and republish information about them and their likeness.
While many nominees
exhibit lifelong heroic behavior, nominators must highlight at least one event
or situation in the last two years.
